Our enrollment process is all through iCare. You can download the app for free in the Apple or Android Store, or use the Google Chrome browser.

Steps in iCare:

Upon Enrollment, your child will go into a ‘waitlist’ until we can review their profile, and ensure we can adequately support their needs.
Helpful Tips:

- Our Enrollment 2026 form is comprehensive at 19 pages long. It will take you approximately 30 minutes to complete. Please be sure to have your child’s medication list readily available as well as list of physicians and their contact information.
- As you work through the form, it is critical that you scroll to the top to click ‘SAVE’ at the VERY top of the Enrollment 2026 form, before you click ‘CLOSE’.

Medical Complexity (Invasive Cares) Enrollment on Hold (g-tube medications, diabetic insulin cares, oxygen, trach needs, or deep suctioning)
At this time, we are waiting for updates from the Medicaid Authorization Team regarding the possibility for licensure for individuals with g-tubes, diabetic cares and other invasive needs such as oxygen or deep suctioning. Due to this, we are hoping to get permission for licensure to be able to support these individuals with nursing in our site, but at this time, a license for nursing does not exist within a care setting. We are working collaboratively with the State and will continue to advocate for this critical care within our center.
Respite Care Requests:
For respite care requests, please select the weekend day(s) you are interested in and indicate your preferred dates. A minimum reservation of three hours is required. We kindly ask that your child remain in care for the full duration of the time you have scheduled.

Uniquely Yours Specialized Care serves individuals with diagnosed developmental or intellectual disabilities who benefit from structured, person‑centered support. To ensure we can provide the appropriate level of care, participants must have a documented disability diagnosis.
At this time, we do not accept siblings without a disability diagnosis. While we recognize that siblings also benefit from breaks, our limited program spaces are reserved for individuals who require specialized support. This helps us maintain proper staffing ratios, provide individualized care, and ensure families of children with disabilities have access to meaningful respite services.

Is my child eligible?
> We serve individuals ages 3–21 with a wide range of support needs.
> Uniquely Yours is designed to support individuals with a wide range of disabilities including autism, intellectual disabilities, developmental delays, and communication or behavioral support needs.
> At this time, we do not accept siblings without a disability diagnosis. What if my child has sensory needs?
> Our team is trained in disability-competent care, sensory support, personal care, and safety. Each child will have a unique set of supports to meet their specific sensory and personal needs.
Do children need to be toilet trained?
> Nope! We can support children with personal care needs, including toileting and diapering, as needed.
How do I know my child will get the right amount of support?
> Part of our enrollment process includes an assessment where you describe your child’s needs. We use this assessment to determine their tier support system placement, making sure each child receives appropriate staffing and care. Children are placed into Tier 1, Tier 2, or Tier 3 based on their level of support needs, supervision requirements, and staffing ratio needed to ensure safety and meaningful participation in programming.
What are your staff to child ratios?
> Ratios vary based on the tier of support, ensuring that that children receive the level of supervision and assistance they need to be safe and successful in programming.
Do you offer 1:1 support?
> Yes. Support levels are based on individualized needs identified during your enrollment assessment.
How will I know how my child’s day went?
> We value strong communication with families and will provide regular updates and communication about your child’s participation and experiences in the program through the iCare app.
Can my child bring assistive technology or communication devices?
> Yes! We encourage children to use their communication devices, assistive technology and other supports that help them participate successfully.
Can my child bring their personal tablet for YouTube or gaming?
> No. At Uniquely Yours, we do not allow personal tablets or gaming devices during programming time. Tablet and gaming devices can be used at home, while our program focuses on building relationships, independence, and community engagement. If your child uses a tablet as a communication device, visual schedule tool, or approved educational supports, those supports will absolutely be incorporated into programming.
Financial and Payment FAQ:
Is there an enrollment fee?
> Yes. There is a one time $50 enrollment fee per child.
How are program rates determined?
> Rates are based on the tier of support your child requires, which is determined during your enrollment assessment. The support tier placement reflects staffing ratios, level of supervision, and individualized support needed for safe and meaningful participation.
Why are the program rates higher than traditional childcare?
> Programs like Uniquely Yours operate with much higher staffing and training requirements because the children we serve often need individualized support for safety, communication, and daily participation. Our goal isn’t just supervision – it’s creating a place where children with complex disabilities can safely play, learn, and build friendships while giving families the support they desperately need.
What if I’m on the waitlist for family support?
> If you are currently on the family support waitlist, you may still be able to access state respite funding through approved respite hours. In addition, we are working to offer scholarship opportunities to help make services more accessible for families when possible.
Do you accept insurance?
> At this time, we do not accept insurance. Our services are private pay, though some families may be able to use state respite funding or other support programs to cover costs.
What if I can’t afford the program?
> We understand the financial challenges many families face. Some families may be able to use: daycare assistance, state respite funding, scholarship opportunities. We are happy to help families explore available funding options.
Do you accept daycare assistance?
> Yes, we accept daycare assistance (DCA) for eligible families to attend our programs. However, families may need to pay some out of pocket costs dependent on scholarship availability.
